Dioscorides and De Materia Medica

An army surgeon's field notes became the standard medical reference for fifteen hundred years — a longer run than any book since, and it worked because he did one thing differently.

What it is

De Materia Medica — "On Medical Material" — compiled around 65 CE by Pedanius Dioscorides, a Greek physician from Anazarbus in Cilicia who served with the Roman army.

Five books, roughly six hundred plants and about a thousand substances in all, each entry giving names in several languages, where the plant grows, how to identify it, how to prepare it, what it is used for, and — unusually — what it can be confused with and how to spot an adulterated sample.

It remained in continuous active use as a reference until the seventeenth century. That is a working life of over fifteen hundred years, and nothing in medicine has come close since.

Where it came from

Dioscorides travelled with the legions, which meant seeing plants across a large part of the Roman world and talking to the people who used them. He says in his preface that he wrote from direct observation rather than from other books, and that he is correcting predecessors who copied one another.

The organising decision is what made it last. Earlier works arranged plants alphabetically. Dioscorides arranged them by what they do — aromatics, oils and salves, animals and their products, grains and vegetables, roots and herbs, and a book on wines and minerals — grouping substances by physiological effect. A physician looking for something to settle a stomach could find a set of candidates together. It is a pharmacological classification rather than a filing system, and it is why the book was usable.

The Vienna Dioscorides, produced around 512 CE for a Byzantine princess, is one of the most important illustrated manuscripts in existence. Its plant illustrations were made to be identifiable in the field — a practical decision that most later copyists abandoned, with the result that medieval herbal illustrations degraded through repeated copying into images nobody could match to a real plant.

Translated into Arabic in the ninth century, it became foundational to Unani pharmacology and was extended by Ibn al-Bayṭār. It returned to Latin Europe through those channels, and the sixteenth-century printed herbals of Brunfels, Fuchs and Mattioli are all substantially commentaries on it.

How it is read

As a reference rather than a treatise — entry by entry, looked up rather than read through. Modern editions are available in English and are surprisingly plain: no theory, no philosophy, just descriptions and uses.

What we can and cannot say

We can say it is a genuinely empirical document by the standards of its time. Dioscorides is notably restrained. He states what a plant is used for, frequently notes disagreement, and occasionally says a claimed use is not credible. He is far less given to magical association than the tradition that followed him — and considerably less than Culpeper fifteen centuries later, who attached a planet to everything.

We can say the identification problem is real. Matching Dioscorides' plants to modern species is difficult and in many cases unresolved. Names shifted, ranges changed, illustrations degraded, and translators substituted local plants for ones they did not know. This is not a historical curiosity: it is the same failure mode that produced the Belgian aristolochia cases in 1991. The question "is this plant the one the text means" has never stopped being dangerous.

We can say some of it holds up. Willow for pain, opium poppy as a narcotic, several genuinely effective purgatives and emetics, and a number of plants still in pharmacopoeias. A good deal more does nothing, and some — mercury and lead preparations among them — is straightforwardly harmful.

And we can say the fifteen-hundred-year run is the most interesting fact about it, and the most double-edged. A book that goes unsuperseded for that long is either extraordinarily good or symptomatic of a field that stopped looking. Both are true here. Dioscorides was the best available observation, and the culture of commentary that grew around him treated the text as the authority and fresh observation as unnecessary. When sixteenth-century botanists finally went back to the plants, they found errors that had been copied faithfully for a millennium.

We cannot ask more of it than its period allowed. What can be said is that it is the single most durable reference work in the history of medicine, that its durability was partly a symptom of stagnation, and that the problem it took most seriously — how do you know this is the right plant — is still the one that kills people.
