- A woody climbing vine to four metres with heart-shaped leaves and slender drooping spikes of small fruit. Black, green and white pepper are the same fruit at different ripeness and processing.

Evidence
Piperine has well-characterised effects on the absorption and metabolism of other substances, which is a real pharmacological finding. Medicinal claims for pepper itself are largely untested.
Safety
Culinary use is benign. The significant issue is the one Ayurveda identified: piperine inhibits drug-metabolising enzymes and the P-glycoprotein efflux pump, raising blood levels of a wide range of drugs. Supplements combining piperine with other actives to boost absorption are common and carry real interaction risk.

Regulatory: Permitted as a food and as a listed medicine ingredient. Piperine-containing enhanced-absorption supplements are widely sold.

Three systems, rating the same plant. Where they disagree, none of them is correcting the others — they are separate frameworks that arrived at their categories independently.
Historical use
The dominant spice of the ancient and medieval trade, moving from Malabar to Rome in bulk. Pliny complained about the drain of Roman silver to India to pay for it. Used medicinally for digestion and, in Ayurveda, explicitly as an enhancer of other herbs.
Recorded as history and attributed where possible. Long use establishes that a plant was widely used, not that it worked, and not that it was safe.
Worth knowing
Ayurveda calls pepper's role yogavahi — a carrier, a substance that enhances the action of what it accompanies — and includes it in trikatu specifically for that purpose. Modern pharmacology calls it a bioavailability enhancer and attributes it to inhibition of CYP3A4 and P-glycoprotein.
The tradition identified a drug interaction and used it deliberately, several centuries before anyone knew what a liver enzyme was. That is one of the clearest hits in the whole collection.
It is also the reason a good many modern supplements are riskier than they look. Adding piperine to raise absorption of a herbal ingredient also raises absorption of whatever prescription medicines the person happens to be taking, and nothing on the label says so.
