Evidence

Trials of red clover isoflavones for menopausal hot flushes have been largely negative; meta-analyses find effects that do not reach clinical relevance. Bone density and cardiovascular claims are unsupported.

Safety

Isoflavones are phytoestrogens with genuine, if weak, oestrogenic activity, which is a real consideration in hormone-sensitive cancers and alongside hormone therapy. Coumarin content raises anticoagulant concerns. Sheep grazing heavily on clover develop documented infertility — clover disease — which is the clearest demonstration that the oestrogenic effect is not trivial.

How the traditions classify it

Historical use

A fodder crop of enormous agricultural importance — nitrogen-fixing clover leys were central to the agricultural revolution. Medicinally used for skin conditions and cough, and included in the Hoxsey formula, a twentieth-century cancer remedy.

Recorded as history and attributed where possible. Long use establishes that a plant was widely used, not that it worked, and not that it was safe.

Worth knowing

Clover disease is the reason to take red clover's oestrogenic activity seriously even though the menopause trials are negative.

In the 1940s, sheep in Western Australia grazing subterranean clover began to suffer widespread infertility, dystocia and permanent reproductive damage. It was traced to the isoflavones — plant compounds that bind oestrogen receptors — and it became one of the foundational demonstrations that phytoestrogens are pharmacologically real.

So the position is unusually specific: red clover isoflavones definitely do something hormonal, and they do not measurably help hot flushes. Those two facts together are more useful than either alone, because they mean a supplement with no demonstrated benefit still carries a genuine reason for caution in anyone with a hormone-sensitive cancer.
