Morphology
An annual legume to about sixty centimetres with three-part leaves and small white-yellow pea flowers, followed by long slender pods of hard angular yellow-brown seeds.

Evidence

Modest evidence for lowering blood glucose in type 2 diabetes, from small trials. Some trials support the lactation claim, though the literature is weak. Effects on testosterone are marketed heavily and poorly supported.

Safety

A food, and well tolerated as one. Consumption produces a strong maple-syrup body odour, which is harmless and has caused false suspicion of maple syrup urine disease in infants of mothers taking it. A legume, so cross-reactivity with peanut and chickpea allergy occurs. Uterine-stimulant reports mean it is avoided in pregnancy at medicinal doses.

Regulatory: Permitted as a food and as a listed medicine ingredient.

Galenic Hot and dry, and used as a poultice and for the chest — the Latin name means Greek hay.
Ayurvedic Bitter and pungent; heating in potency. Used for digestion and, traditionally, to promote milk.

Historical use

Among the oldest cultivated plants, found in Bronze Age deposits and in Tutankhamun’s tomb. Used across the Mediterranean, the Middle East and South Asia as food, spice, fodder and medicine, and consistently associated with lactation.

Recorded as history and attributed where possible. Long use establishes that a plant was widely used, not that it worked, and not that it was safe.

Worth knowing

Fenugreek's most reliable effect is the one nobody sells it for: it makes people smell of maple syrup. Sotolone, the compound responsible, is the same one that gives maple syrup its characteristic smell, and it is excreted in sweat and urine within hours.

This is not a curiosity. Infants of mothers taking fenugreek for lactation have been investigated for maple syrup urine disease — a serious inherited metabolic disorder diagnosed partly by smell — on the strength of it. A herb that produces a false positive for a genetic disease is a good illustration of why clinicians want to know what people are taking.
